Before agreeing to participate in this research study, it is important that you read and understand the following explanation of the proposed procedures. It describes the procedures, benefits, risks, and discomforts of the study. It also describes your right to withdraw from the study at any time. It is important for you to understand that no guarantees or assurances can be made as to the results of the study.

Purpose of the study and how long it will last

The purpose of administration of the survey is the tracking of technology skill and attitudes. It will take about 15 minutes to complete the survey each time at the beginning and end of the school year.

Description of the study including the procedures to be used

This survey asks about your use and attitudes of technology in the classroom. It shouldn't take more than 15 minutes of your time to answer the questions.

Description of procedures/elements that may result in discomfort or inconvenience

There are no foreseeable physical or psychological risks to you as you complete the online survey.

Description of the procedures/elements that are associated with foreseeable risks

There are no foreseeable risks in completing the online survey.

Benefits to the subjects or others

This evaluation will allow the school district to measure the success of the professional development efforts and the growth of their teaching force's technical skills and use. Your involvement in this study is instrumental in tracking the success of this project. The inclusion of technology in classrooms could have significant educational benefit to students and provide teachers with more effective teaching skills and strategies.

Confidentiality of research records

Your answers to the survey will be kept on a secure server at the University of North Texas. Only the researchers and their research assistants will have access to the data using a secure password. Since this study requires correlation between your answers at the beginning and end of the study you will be asked to create a user name consisting of numbers and letters known only to you. No attempt will be made to identify you individually.

Written and oral reports will never contain information about an individual person nor will any person be identified in such reports.

Withdrawal from Study

You are free to withdraw your consent and discontinue participation in the study at any time without consequences. There may be unforeseen circumstances that develop under which your participation may be terminated by the investigator.

Payment for participation in the research

Participants in this study receive no payment for completing the survey.

Legal Rights

You are NOT waiving any legal rights by acknowledging that you have read this consent form.

Review for protection of participants

This research study has been reviewed and approved by the UNT Committee for the Protection of Human Subjects 940-565-3940.

I understand that I do not have to take part in this study, and my refusal to participate or my decision to withdraw will involve no penalty or loss of rights or benefits or legal recourse to which I am entitled. The study personnel may choose to stop my participation at any time.

By completing the survey I signify that I understand my rights as a research subject, and I voluntarily consent to participate in this study. Additionally, I understand what the study is about and how and why it is being done. You may print a copy of this consent form for your records.
