Instructions for Copying and Pasting a Form into an Email

If you have a PC or Mac:

  • Select everything on the page by holding down the control/command key and pushing "a"
  • Copy the document by holding down the control/command key and pushing "c"
  • Click on the link to send email - the email will be addressed to the correct person
  • When the email form is ready, paste the document into the body of the email by holding down the control/command key and pushing "v"
  • Answer the questions and add comments to the form and then send it
